can provide you with information, resources, and

The Care Coordinator will work for you, enabling
you to clear your schedule and your mind,
so you can
enjoy your time with your aging parent.

Care Coordination
| Our Care Coordinator
will assist you and your family in coordinating the various
care options
for your
aged parent and your family. The Care Coordinator
will assist in helping your family establish the goal
for care for your aged parent. If your parent’s needs change
over time, the Care Coordinator will assist
you in developing new plans and new goals. |
| Need help coordinating
decision making? In many
cases, there are family members living both within the area,
and at
a distance.
Communications
between these family members can be stressful when it comes to
making decisions while caring for an
aged parent. There are times when an aged parent will not be
entirely truthful with their adult children,
but will speak candidly with a non-family member. There may be
varying opinions within your family
concerning how best to care for an aging parent. Through family
meetings and individual contact, the
Care Coordinator will work with you to gain a consensus for the
appropriate care for your aging parent. |
| Ensure family goals are met. If Assisting Aging
Parents is the primary provider, our Care Coordinator will
monitor
your
aging
parent
through phone calls or home visits, to ensure that your family’s
goal continues to be met and will report
results regularly. If your family determines that your aging
parent needs to be moved to a care facility,
the Care Coordinator can visit and evaluate these facilities.
The Coordinator will provide the
information to your family and assist in determining the most
appropriate facility for your loved one. |
| These services are billed at $50 per hour with
a one hour minimum. Each quarter hour thereafter is
$12.50. |
